Admission Forms and CCS Handbook

Below you will find the paperwork and forms required for New and Returning students.
For Parents and Guardians
Please download and READ the Capital Christian School Handbook 2024-2025.
You can also print a copy of the Northern New England Conference Academic Calendar for the 2024 - 2025 school year.
For New Students
It is especially important for New Student families to download and READ the Capital Christian School Handbook 2024-2025.
Please download and print this New Student Check List for guidance in filling out the forms.
To apply for admission: Download and print the New Student Application Form.
Fill out and return to CCS, with $25.00 Application Fee, ASAP. All entrance fees must be paid in full prior to the first day of school. Please mail the completed admission form, along with the $25.00 application fee.
If your child is transferring to CCS from a previous school, you will need to provide their transcripts. You can download and print this Request to Release Records form.
Transcripts must be received before being considered officially enrolled.

For Returning Students
You should have received a Returning Student Packet for 2024 - 2025 from the school. If you need replacement forms, you can find those forms on this page.
Please download and print this Returning Student Checklist 2024 - 2025 for guidance in filling out the forms.
To apply for admission: Download and print the Returning Student Application Form 2024 - 2025.
Fill out and return to CCS ASAP. All entrance fees must be paid in full prior to the first day of school.

Computer Acceptable Use Policy and Internet Agreement (You keep the Acceptable Use policy, bring the Internet Agreement to registration)
